What it is, How it works

A 1.5-inch segment of roughly 200 hair strands (equivalent to the size of a #2 pencil) nearest to the scalp will produce 100mg of hair, which is the ideal sample amount for screening and confirmation. For EtG, additional tests, and/or those exceeding 10 panels, 150mg of the sample is advisable. We suggest weighing the sample on a jeweler’s scale. If scalp hair is unavailable, a corresponding amount of body hair might be obtained. When mentioning head hair, we are specifically discussing scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result primarily involves four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial incorporation of a sample into a laboratory’s system. This involves checking that the sample was sealed and shipped correctly, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry not supplied by an electronic chain of custody system.

Screening encompasses a preliminary check for drugs of abuse. Although Screening is an economical method to exclude drug usage in most samples, a positive screen necessitates confirmation to be valid in court. Any samples indicative of a positive result in Screening require a secondary confirmation.

If a sample is indicative of a positive result in the Screening phase, additional hair is drawn from the initial sample and prepared for Extraction. During this phase, drugs are extracted from hair at considerably lower concentrations compared to other methods (ex. urine or oral fluid), making hair drug testing the most challenging method to execute.

Should a positive screening result occur, Confirmation is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. All samples indicative of a positive result undergo washing before confirmation if necessary. The complete laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
