What it is, How it works

When taken from near the scalp, hair can present an approximate 3-month timeframe for detecting alcohol and drug use. Hair is straightforward to gather, challenging to tamper with, and simple to transport.

A 1.5-inch section comprising around 200 strands of hair (roughly the thickness of a #2 pencil) close to the scalp will yield 100mg of hair, the best sample size for screening and validation. For EtG, supplemental tests, and/or analyses exceeding 10 panels, 150mg of the specimen is advised. We suggest weighing the sample with a jeweler’s scale. If scalp hair is unavailable, an equivalent amount of body hair may be gathered. When talking about head hair, we mean scalp hair only. Body hair pertains to all other types of hair (facial, axillary, etc.).

Process Overview

The four crucial stages in the laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial entry of a sample into a laboratory’s workflow. This step involves ensuring the sample was sealed and shipped correctly, generating a random LAN (Laboratory Accessioning Number), and performing any supplementary data entry absent from an electronic chain of custody system.

Screening provides a preliminary rapid evaluation for drug abuse. Although Screening is an affordable approach to exclude drug use in the majority of cases, a positive screen must be validated to be acceptable in judicial contexts. Samples that test presumptively positive in Screening warrant an additional confirmation.

Should a sample test presumptively positive during Screening, additional hair is extracted from the original sample and readied for Extraction. During this phase, drugs are isolated from hair at far lower concentrations than in other methodologies (e.g., urine or oral fluid), making hair drug screening the most demanding technique to execute.

Validation of any positive screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to validation as necessary. The full laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the compliance with ISO / IEC 17025 standards.
