Hair collection is straightforward, somewhat challenging to tamper with, and convenient to transport.

Acquiring a 1.5-inch segment of around 200 hair strands (comparable to a #2 pencil size) near the scalp yields 100mg of hair, optimal for screening and validation. For EtG, additional tests, and/or tests beyond 10 panels, a mass of 150mg is suggested. It's advisable to weigh the hair sample using a jeweler’s scale. If scalp hair isn't available, a similar quantity of body hair can be obtained. References to head hair imply scalp hair solely. Body hair is inclusive of all other hair categories (such as facial, axillary, etc.).

Process Overview

Laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of entering a sample into the laboratory's system. This step includes checking that the sample was properly sealed and shipped, assigning a randomly generated LAN (Laboratory Accessioning Number), and entering any supplementary data not captured by an electronic chain of custody system.

Screening involves an initial swift assessment for drugs of abuse. Although Screening is a cost-efficient method to eliminate drug use on most samples, a positive screen result needs a confirmation to be court-admissible. Samples that are presumptively positive during Screening demand secondary confirmation.

If the Screening process indicates a presumptive positive result, additional hair from the original sample is used for Extraction. During this phase, drugs are isolated from the hair, extracted at lower concentrations than other methods (such as urine or oral fluid), establishing hair drug screening as a particularly complex technique.

The Confirmation of any positive screen result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All samples that show presumptive positive results undergo washing before Confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is inspected under the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field.
