What it is, How it works

Hair records a prolonged account of alcohol and drug use by capturing biomarkers in the hair's fibers as it grows. When obtained near the scalp, hair can offer a detection period of up to around 90 days for alcohol and other drugs. Hair collection is straightforward, relatively tough to tamper with, and simple to transport.

A 1.5-inch segment, containing roughly 200 strands of hair (comparable to the thickness of a #2 pencil) nearest to the scalp, yields 100mg of hair, an ideal sample for testing and confirmation processes. For EtG, additional panels, and/or evaluations exceeding 10 panels, 150mg of the sample is advised. Using a jeweler’s scale to weigh the specimen is recommended. If scalp hair isn't available, a comparable quantity of body hair may be obtained. References to head hair imply only scalp hair, while body hair encompasses all other types (including facial, axillary, etc.).

Process Overview

There are four primary phases in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial integration of a specimen into a lab's system. This step checks that the specimen was securely sealed and correctly shipped, allocates a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry not covered by an electronic chain of custody system.

Screening consists of a quick preliminary evaluation for drugs of abuse. While Screening is a cost-efficient approach to rule out drug use for most specimens, a positive screen must be validated for court acceptance. Specimens that register presumptive positives in Screening require further verification.

If Screening detects a presumptive positive, additional hair is drawn from the original specimen for the Extraction phase. Here, drugs are isolated from hair at significantly lower concentrations than through other methods (such as urine or oral fluid), reflecting the complexity of hair drug screening.

Confirmation of any positive screening outcome is completed using GC/MS, GC/MS/MS, or LC/MS/MS methods. Presumptive positive specimens undergo washing prior to confirmation as necessary. The entire laboratory workflow from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and in accordance with ISO / IEC 17025 standards.

Clinical Applications of hair testing are witnessed by the insight on environmental tobacco smoke exposure, assessed by hair analysis in a population of students and the study of in utero-exposure to drugs of abuse through neonatal hair analysis. Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
