What it is, How it works

Hair testing is increasingly recognized as an effective tool for detecting drug and alcohol consumption. Through the trapping of biomarkers in the fibers of the growing hair strand, hair can provide a long-term history of alcohol and other drugs. When taken near the scalp, hair can offer a detection period of up to about three months for alcohol and other drugs. Hair is easy to gather, hard to tamper with, and convenient to transport.

A 1.5-inch segment of approximately 200 hair strands (roughly the diameter of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the ideal sample volume for screening and confirmation tests. For EtG, extra tests, and/or tests exceeding 10 panels, a 150mg specimen is advised. Using a jeweler’s scale to weigh the sample is recommended. If scalp hair is not available, the same amount of body hair can be used. When mentioning head hair, we refer specifically to scalp hair. Body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

There are four primary steps in processing a drug test result in the lab: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a sample into the lab's system. This step includes ensuring the sample was sealed and sent correctly, granting a random LAN (Laboratory Accessioning Number), and finalizing any needed data entry absent from an electronic chain of custody system.

Screening entails a preliminary quick examination for drugs of abuse. While Screening is an economical method to exclude drug usage in the majority of samples, a positive screen necessitates confirmation for court admissibility. Any samples presumed positive in Screening require secondary confirmation.

If a sample is deemed presumptively positive during Screening, more hair is taken from the original specimen and readied for Extraction. In this phase, drugs are removed from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), highlighting why hair drug screening is the most challenging methodology to conduct.

Confirmation of a positive screening outcome is approached through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all samples assumed positive undergo washing as necessary. The entire lab process from Accessioning to Confirmation adheres to review under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
