Hair testing has emerged as an effective means for identifying drug and alcohol consumption. Hair captures a historical record of alcohol and drug intake by embedding biomarkers within the fibers of the growing hair strands. When gathered near the scalp, hair can reveal a detection range for alcohol and drugs stretching up to about 3 months. Collecting hair is straightforward, challenging to tamper with, and convenient to send off.
A sample measuring 1.5 inches and consisting of approximately 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the ideal amount for both screening and confirmation. For EtG, add-ons, or tests exceeding 10 panels, 150mg of the hair is advised. It is best to weigh the sample using a jeweler's scale. Should scalp hair be inaccessible, an equivalent quantity of body hair may be gathered. When discussing head hair, we specifically mean scalp hair. Body hair includes all other hair types (facial, axillary, etc.).
Process Overview
The laboratory procedure for processing drug test results consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.
Accessioning involves the initial entry of a sample into a laboratory's system. This step includes confirming that the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data processing not managed by an electronic chain of custody system.
Screening consists of an initial rapid evaluation for drugs of concern. While Screening is an economical method to dismiss drug use in the majority of samples, a positive screen must be validated to be accepted in legal proceedings. Any samples showing presumptive positivity in Screening must undergo further confirmation.
Should a sample appear presumptively positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than with other methods (such as urine or oral fluid), which is why hair drug screening is considered a complex methodology.
Confirmation of any positive Screening result is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed prior to confirmation as necessitated. The whole laboratory sequence from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
