What it is, How it works

Hair captures a chronological timeline of alcohol and drugs by embedding biomarkers in the strands as they grow. When taken near the scalp, hair can reveal up to an estimated 3-month period of alcohol and drug usage. Hair samples are straightforward to collect, relatively tamper-resistant, and simple to send.

A sample of 1.5 inches of approximately 200 hair strands (comparable in size to a #2 pencil) located nearest to the scalp yields 100mg of hair, which is the optimal sample for initial and confirmatory testing. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale for weighing the sample. If scalp hair cannot be obtained, a similar amount of body hair can be used. When mentioning head hair, we refer specifically to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial incorporation of a specimen into a laboratory’s system. This step involves confirming the specimen was sealed and dispatched correctly, assigning it a unique LAN (Laboratory Accessioning Number), and completing any extra data inputs not covered by an electronic chain of custody.

Screening is a preliminary cursory examination for drug presence. At this stage, drugs are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), establishing hair drug screening as one of the most complex methods to execute.

Confirmation of positive screened results is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleansed before confirmation as warranted. The entire laboratory procedure from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.
