For EtG, additional tests, and/or panels exceeding 10, a 150mg sample is preferred. Measuring the weight of the sample is recommended using a jeweler's scale. If scalp hair can't be obtained, an equivalent amount of body hair may serve as a substitute. Reference to head hair implies only scalp hair, whereas body hair encompasses all other types (e.g., facial, axillary).

Process Overview

The laboratory procedures for processing a drug test result include four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase where a sample is integrated into a lab's system. This step ensures the sample was correctly sealed and delivered, assigns a unique LAN (Laboratory Accessioning Number), and completes any additional data input absent from an electronic custody chain.

Screening provides a preliminary analysis for substances of abuse. Though cost-effective for eliminating drug presence in most cases, a positive Screening must be confirmed to hold up in court. Samples flagged as presumptively positive in Screening necessitate a follow-up confirmation.

Should a sample show presumptive positivity in Screening, additional hair is extracted from the original sample and readied for Extraction. During Extraction, drugs are retrieved from hair at a lower concentration compared to other methods (like urine or oral fluid), underscoring why hair drug screening is the most challenging methodology.

Confirmation of any positive result in Screening is executed through GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, presumptive positive samples are cleansed as required. From Accessioning to Confirmation, the entire laboratory process complies with both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
