What it is, How it works

When collected near the scalp, hair offers up to around a 3-month timeframe for detecting both alcohol and drugs. Collection of hair is straightforward, relatively challenging to tamper with, and hassle-free to transport.

A 1.5-inch segment encompassing about 200 hairs (approximately the diameter of a #2 pencil) close to the scalp will yield 100mg of hair, which is the optimal amount for both screening and confirmation purposes. For EtG, additional panels, and/or tests beyond 10 panels, a 150mg specimen is advised. Using a jeweler’s scale for specimen weighting is advisable. Should scalp hair be unavailable, a corresponding amount of body hair can be utilized. The term head hair denotes only scalp hair. Body hair encompasses all other varieties (such as facial, axillary, etc.).

Process Overview

The laboratory process for a drug test result encompasses four core steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial phase of integrating a sample into a laboratory’s system. This involves confirming that the sample was correctly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and executing any extra data entry absent from an electronic chain of custody system.

Screening constitutes a preliminary check for drugs of abuse. While a cost-effective measure to eliminate drug use in the majority of samples, a positive screen must be validated for admissibility in legal proceedings. Any samples showing a presumptive positive during Screening require a secondary confirmation.

Should a sample exhibit a presumptive positive in Screening, additional hair is extracted from the original specimen and prepped for Extraction. Here, drugs are extracted from hair at notably lower concentrations compared to other methodologies (such as urine or oral fluid), rendering hair drug screening as the most intricate methodology to undertake.

Confirmation of any positive screening outcomes is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as necessary. The entire lab procedure from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
