Hair analysis has emerged as an effective method for identifying substance use. Hair holds a prolonged history of alcohol and drug consumption by encapsulating biomarkers within the threads of the expanding hair shaft. If taken near the scalp, hair can deliver up to a three-month detection duration for alcohol and drugs. Hair collection is straightforward, moderately challenging to tamper with, and easy to distribute.
A 1.5-inch segment of about 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is optimal for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, it is advised to gather 150mg of the specimen. Measuring the specimen with a jeweler's scale is recommended. If scalp hair is not accessible, an equivalent quantity of body hair can be used. References to head hair imply scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).
Process Overview
The key phases in lab analysis of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.
Accessioning involves initial sample processing into a lab's system, which includes confirming the sample was sealed and transported correctly, allocating a random LAN (Laboratory Accessioning Number), and fulfilling any extra data entry absent in an electronic chain of custody system.
Screening offers an initial rapid assessment for drugs of abuse. While Screening is economical to exclude drug use in most samples, a positive screen must be validated to be admissible in legal proceedings. Samples with presumptive positives in Screening require a secondary verification.
When a sample shows a presumptive positive at the Screening phase, more hair is drawn from the initial sample and readied for Extraction. In this phase, substances are withdrawn from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening the most complex method to execute.
Confirmation of any positive Screening result is achieved using GC/MS, GC/MS/MS, or LC/MS/MS. All samples presumed positive are cleaned before confirmation as necessary. The full lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

Note: While often termed "hair follicle tests", this assessment evaluates the hair strand, not the follicle beneath the scalp.
