A 1.5-inch segment of around 200 hair strands (roughly the circumference of a #2 pencil) near the scalp provides 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a specimen amount of 150mg is suggested. We advise using a jeweler’s scale to weigh the specimen. If scalp hair is unobtainable, a matching amount of body hair is an alternative. Head hair exclusively refers to scalp hair. Body hair comprises all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test outcome involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning incorporates the initial processing of a sample into a laboratory’s system. This process involves confirming that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and filling in any supplementary data entry absent in an electronic chain of custody system.

Screening includes an initial rapid assessment for drug abuse. While screening is a cost-efficient means to exclude drug usage for the majority of samples, a positive screen must be confirmed to be permissible in legal environments. Any samples that are presumptively positive in Screening necessitate a secondary confirmation.

If a sample is assumed positive in the Screening phase, additional hair is extracted from the initial specimen and prepared for Extraction. In this phase, drugs are extracted from hair at significantly lower concentrations than through other methodologies (e.g., urine or oral fluid), which renders hair drug screening the most demanding methodology to conduct.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples undergo washing prior to confirmation as necessary. The comprehensive laboratory process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
