What it is, How it works

Hair evaluations have become recognized as a robust means for identifying drug and alcohol consumption. Hair offers a prolonged record of alcohol and drugs by capturing biomarkers within the fibers as the hair grows. When hair is sampled near the scalp, it can reveal up to roughly a 3-month window for detecting alcohol and drug use. Hair is straightforward to collect, relatively challenging to tamper with, and simple to transport.

A 1.5-inch segment of roughly 200 hair strands (approximately the size of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the ideal amount for both screening and confirmation. For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the hair sample is advised. It is suggested to weigh the specimen using a jeweler’s scale. If scalp hair is not available, a comparable volume of body hair may be utilized. When we mention head hair, it specifically refers to scalp hair. Body hair encompasses all other types of hair (facial, axillary, etc.).

Process Overview

The four primary stages in the lab processing of a drug test include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial integration of a sample into the laboratory’s system. This process includes verifying that the sample was correctly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of misuse. While Screening offers a cost-efficient method to exclude drug consumption on most samples, a positive screen requires confirmation to be considered acceptable in court. Any samples that are tentatively positive in Screening necessitate a further confirmation.

If a sample shows tentative positivity during the Screening phase, additional hair is taken from the original sample and prepared for Extraction. At this point, drugs are separated from hair at significantly lower concentrations compared to other methods (such as urine or oral fluid), which explains why hair drug screening is one of the most challenging methods to execute.

Confirmation of any positive screening outcome is carried out through GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positivity are cleaned prior to confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 requirements.
