What it is, How it works

The utilization of hair testing has gained recognition as an effective method for identifying drug and alcohol consumption. Hair offers an extended history of alcohol and other drugs due to the entrapment of biomarkers within the fibers of the developing hair strand. When taken near the scalp, hair can reveal up to an estimated 3-month period for detecting alcohol and other substances. Hair collection is straightforward, difficult to tamper with, and convenient to transport.

A sample measuring 1.5 inches, consisting of about 200 hair strands (approximately the thickness of a #2 pencil) nearest to the scalp, yields 100mg of hair, which is the perfect quantity for both screening and confirmation. For EtG, supplementary tests, and/or panels exceeding 10, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the sample. If scalp hair isn't accessible, a similar quantity of body hair may be obtained. Head hair specifically refers to scalp hair. Body hair encompasses all other hair types, including facial, axillary, etc.

Process Overview

The processing of a drug test result in the laboratory involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample within the laboratory system. It entails verifying proper sealing and shipment of the sample, assigning a random Laboratory Accessioning Number (LAN), and completing any additional data entry not furnished by an electronic chain of custody system.

Screening executes an initial rapid assessment for substances of abuse. While Screening serves as a cost-efficient method to dismiss drug usage for most samples, results necessitating court admissibility must undergo confirmation. A secondary confirmation is required for any samples that are presumptively positive during Screening.

Should a sample test presumptively positive in the Screening phase, additional hair is extracted from the initial sample and readied for the Extraction stage. During this phase, drugs are extracted from hair at significantly lower concentrations than other techniques (e.g., urine or oral fluid), establishing hair drug screening as the most challenging methodology to execute.

Confirmation for any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all presumptive positive samples are washed as necessary. The comprehensive laboratory procedure from Accessioning to Confirmation is assessed in alignment with both the CAP (College of American Pathologists) Hair specification and the ISO / IEC 17025 accreditation standards.

Advantages of hair drug testing:

  • Extended detection period: Hair drug screens can identify drug use for up to 90 days, whereas urine tests offer a shorter detection period.
  • Challenging to deceive: Hair drug tests are notably difficult to manipulate, enhancing result accuracy.
  • Historical use insights: Allows assessment of drug usage patterns over time, beyond recent consumption.

Limitations:

  • Inability to detect recent use: Detection in hair requires approximately 5-7 days post-drug use.
  • Expense: Hair drug screenings tend to be pricier than other drug testing techniques.
  • Result variability: Elements like hair color and individual hair growth differences can influence drug metabolite concentrations in hair.

Note: Commonly termed as "hair follicle tests," this testing analyzes the hair strand, not the follicle beneath the scalp.
