Process Overview

The laboratory processing of a drug test result encompasses four primary phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the laboratory's system. This entails checking the sample's seal and ensuring correct shipment, allocating a random LAN (Laboratory Accessioning Number), and completing any extra data entry not covered by an electronic chain of custody.

Screening entails a preliminary quick check for substances of abuse. While Screening is economically efficient for ruling out drug use in most samples, a positive result necessitates confirmation to be valid in legal proceedings. Presumptive positives in Screening require additional verification.

Should a sample present a presumptive positive during Screening, further hair from the original specimen is utilized for Extraction. In this phase, drug extractions from hair occur at significantly lower levels compared to other methods (ex. urine or oral fluid), making hair drug screening particularly challenging to execute.

Positive Screening outcomes undergo Confirmation through methods such as GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are typically cleansed before Confirmation when necessary. The entire procedure from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair criteria and ISO / IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
