What it is, How it works

Hair testing has become recognized as a robust method for identifying the consumption of drugs and alcohol. The strands of hair capture biomarkers in their fibers, offering a long-term record of substance use. When hair is taken near the scalp, it can offer a detection period of about three months for alcohol and other substances. Collecting hair is straightforward, moderately challenging to tamper with, and simple to send.

A sample measuring 1.5 inches of around 200 hair strands (roughly the diameter of a #2 pencil) near the scalp provides 100mg of hair, the preferred quantity for screening and verification. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advised. Using a jeweler’s scale for weighing the sample is suggested. If scalp hair cannot be obtained, an equivalent amount of body hair can be gathered. When we refer to head hair, it specifically means scalp hair. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory process for a drug test outcome encompasses four primary phases: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the introduction of a sample into the lab’s system. This step ensures that the sample was adequately sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and finalizes any extra data entry absent from an electronic chain of custody system.

Screening entails a preliminary rapid assessment for drugs of abuse. Although Screening is economically viable to rule out drug use for most samples, a positive result must be confirmed to stand in court. Samples that are provisionally positive during Screening necessitate a secondary validation.

If a sample yields a provisional positive during the Screening phase, more hair is taken from the initial sample and prepped for Extraction. In this stage, drugs are removed from the hair at significantly lower concentrations than other methods (like urine or oral fluid), which makes hair drug screening the most challenging method to execute.

Validation of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as required. The complete lab process from Accessioning to Confirmation is examined under the CAP (College of American Pathologists) Hair designation and complies with ISO / IEC 17025 accreditation standards.
