What it is, How it works

Hair testing is now recognized as a potent method for detecting drug and alcohol consumption. Hair serves as a chronicled record of alcohol and drug intake by entrapping biomarkers within the hair's growing structure. When obtained near the scalp, hair can present roughly a 90-day detection span for alcohol and drugs. Hair is straightforward to collect, relatively challenging to tamper with, and convenient to transport.

A 1.5-inch segment comprising about 200 strands of hair (similar in thickness to a #2 pencil) closest to the scalp yields 100mg of hair, the preferred sample size for both screening and validation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is suggested. We advocate weighing the sample on a jeweler’s scale. If scalp hair is inaccessible, a comparable quantity of body hair can be used. By "head hair," we mean scalp hair exclusively. "Body hair" encompasses all other hair types (facial, underarm, etc.).

Process Overview

The core steps in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the preliminary processing of a sample into a laboratory's system. This step includes confirming that the sample was properly sealed and transported, assigning an arbitrary LAN (Laboratory Accessioning Number), and completing any supplemental data entry absent in an electronic chain of custody format.

Screening entails an initial rapid assessment for illicit drugs. Although Screening is a cost-efficient method to exclude drug usage on most samples, a positive screening result needs verification to be legally viable. Any samples deemed presumptively positive during Screening necessitate further confirmation.

In the event that a sample is presumptively positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. During this phase, drugs are isolated from the hair at a significantly lower concentration than in other methods (such as urine or oral fluid), rendering hair drug screening the most challenging technique to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptively positive are washed prior to confirmation as needed. The comprehensive laboratory processing from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
