For EtG, additional tests, and/or tests exceeding 10 panels, 150mg of the sample is advised. It is suggested to measure the sample using a jeweler’s scale. Should scalp hair be unavailable, a matching volume of body hair may be collected. When discussing head hair, it specifically pertains to scalp hair. Body hair encompasses other hair types (such as facial or axillary hair).

Process Overview

There are four principal steps in the laboratory handling of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial processing of a specimen into the lab's system. This process verifies the sample’s proper sealing and delivery, assigns a random LAN (Laboratory Accessioning Number), and completes any remaining data entry not covered by an electronic chain of custody system.

Screening provides an initial rapid check for drug misuse. Although Screening offers an economical method to exclude drug use in most samples, a positive result requires confirmation to hold up in court. All samples showing presumptive positive results in Screening need further confirmation.

Should a sample show a presumptive positive outcome in the Screening phase, additional hair is taken from the original specimen for Extraction. This phase extracts drugs from hair at a much lower concentration compared to other methods (e.g., urine or oral fluid), which is why hair drug analysis is considered the most challenging technique to conduct.

Verification of any positive Screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Every presumptive positive sample undergoes washing before confirmation as necessary. The entire laboratory workflow from Accessioning to Confirmation is overseen under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
