What it is, How it works

Hair examinations are now recognized as a robust method for identifying drug and alcohol consumption. Hair retains an extended record of alcohol and drugs by encapsulating biomarkers in the strands of the growing hair. When collected near the scalp, hair can provide nearly a 3-month period for detecting alcohol and drugs. The collection process for hair is straightforward, somewhat hard to tamper with, and simple to send.

A 1.5-inch segment comprising about 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, an optimal sample size for both screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of the sample is suggested. Using a jeweler’s scale to weigh the sample is advisable. If scalp hair is unavailable, an equivalent amount of body hair may be taken. Notably, when mentioning head hair, it refers exclusively to scalp hair. Body hair pertains to all other types (facial, axillary, etc.).

Process Overview

The four essential steps in the laboratory analysis of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the preliminary processing of a sample into the laboratory’s database. It ensures the sample is properly sealed and dispatched, assigns a unique LAN (Laboratory Accessioning Number), and fulfills any extra data entry not supplied by an electronic chain of custody system.

Screening conducts an initial rapid assessment for substances of abuse. Though Screening provides a cost-effective approach to dismiss drug usage on most samples, a positive result in Screening must be confirmed to be valid in legal contexts. Presumptive positives in Screening necessitate a subsequent confirmation.

Should a sample appear presumptively positive at the Screening phase, additional hair is collected from the original sample and readied for Extraction. In Extraction, drugs are obtained from hair in much lower concentrations than in other methods (e.g., urine or oral fluids), resulting in hair drug screening being the most challenging methodology to execute.

Confirmation of any positive Screening finding is accomplished via GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptively positive samples undergo washing prior to confirmation as required. The complete laboratory procedure from Accessioning to Confirmation undergoes review under both CAP (College of American Pathologists) Hair designation and accreditation according to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
