Process Overview

Laboratory processing of a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the process of entering the sample into the laboratory's system. This phase involves ensuring the sample's seal and shipment are intact, allotting a LAN (Laboratory Accessioning Number) randomly, and filling in any missing data not available in an electronic chain of custody.

The Screening phase serves as an initial test for the presence of drugs. Although cost-effective for negating drug use in most cases, a positive Screening result necessitates confirmation to be admissible in legal contexts. Presumptive positives from Screening require further confirmation.

When a Screening test suggests a positive, additional hair is taken from the original sample to prepare for Extraction. This process involves retrieving drugs from hair at lower concentrations than methods such as urine or oral fluid tests, underlining the complexity of hair drug screening.

Positive samples from Screening undergo Confirmation through methods such as GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, any presumptive positive samples are cleaned as necessary. The entire process, from Accessioning to Confirmation, adheres to both CAP (College of American Pathologists) Hair standards and accreditation under ISO / IEC 17025.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
