For EtG, additional tests, and/or assessments exceeding 10 panels, 150mg of the sample is suggested. It is advisable to weigh the sample using a jeweler’s scale. If scalp hair isn’t available, an equivalent quantity of body hair can be used. When mentioning head hair, it refers solely to scalp hair. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step of integrating a sample into a lab's system. This step includes confirming that the sample was correctly sealed and transported, giving a random LAN (Laboratory Accessioning Number), and entering any extra data not included by an electronic custody system.

Screening is a preliminary rapid check for substances of abuse. Although Screening is an economical method to eliminate drug usage in most samples, any positive screen must be confirmed to be court-admissible. Any samples that are initially positive in Screening need additional confirmation.

If a sample is initially positive during the Screening, more hair is taken from the original sample and readied for Extraction. In this stage, substances are extracted from hair at significantly lower concentrations than other methodologies (e.g., urine or oral fluid), making hair drug screening the most challenging methodology to execute.

Confirmation of any positive screening outcome is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives are washed before confirmation if needed. The total laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
