For EtG enhancements or tests surpassing 10 panels, a 150mg sample is advisable. Utilize a jeweler's scale for precise specimen measurement. Lacking scalp hair, an equivalent amount of body hair may suffice. Head hair denotes exclusively scalp hair, whereas body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory progression of a drug test encompasses four crucial steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial assimilation of a sample into the lab system. This step verifies correct sealing and shipping, assigns a unique LAN (Laboratory Accessioning Number), and completes any outstanding data entry omitted by an electronic chain of custody system.

Screening executes a preliminary assessment for substance abuse. This economical approach excludes drug use in most cases, yet positive readings necessitate confirmation for legal validity. Samples indicating positive results in Screening mandate subsequent confirmation.

Should a sample indicate positivity during the Screening phase, additional hair is retrieved from the original batch for Extraction. Here, substances are extracted from hair at markedly lower concentrations compared to alternative methods (e.g., urine or oral fluid), rendering hair drug analysis as the most challenging procedure.

Confirmation of positive results employs GC/MS, GC/MS/MS, or LC/MS/MS. Prior to confirmation, all tentative positive samples undergo necessary washing. Total laboratory procedures from Accessioning to Confirmation are conducted under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
