A sample measuring 1.5 inches and consisting of around 200 hair strands (roughly the width of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the perfect quantity for testing and verification. For EtG, additional assessments, and/or evaluations exceeding 10 panels, 150mg of the hair is suggested. It's advisable to weigh the hair on a jeweler’s scale. If head hair is unavailable, an equivalent amount of body hair may be used instead. When we mention head hair, it specifically refers to scalp hair. Body hair encompasses other types including facial and axillary hair.

Process Overview

The principal stages involved in processing drug test results in a laboratory include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab's systems. This step involves checking that the sample was correctly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any further data input not covered by an electronic chain of custody system.

Screening entails a rapid initial evaluation for substances of misuse. Although Screening is an economical approach to exclude drug consumption in most samples, a positive screen requires confirmation to be legally viable. Any samples with a presumptive positive in Screening must undergo additional confirmation.

If a specimen shows a presumptive positive during Screening, further hair is extracted from the initial sample for preparatory Extraction. In this phase, drugs are drawn out from hair at a much lower concentration compared to other methods (such as urine or oral fluid), making hair drug analysis the most challenging methodology to execute.

Confirmation of all positive screening outcomes is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Any samples with presumptive positives are washed before confirmation if necessary. The full laboratory process, from Accessioning to Confirmation, is reviewed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.
