What it is, How it works

Hair provides an extended history of alcohol and drugs by capturing biomarkers in the fibers of the growing hair strand. If gathered near the scalp, hair can offer up to about a 3-month detection period for alcohol and other drugs. Hair collection is straightforward, somewhat resistant to tampering, and easy to send.

A sample of 1.5 inches consisting of roughly 200 hair strands (about the size of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimum sample for both screening and confirmation. For EtG, add-ons, and/or tests beyond 10 panels, 150mg of the specimen is advisable. It is suggested to measure the specimen with a jeweler’s scale. If there is no scalp hair, an equivalent amount of body hair might be collected. When discussing head hair, we mean scalp hair only. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The four primary steps in laboratory processing of a drug test result are Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into a lab’s protocol. This involves confirming the sample was properly sealed and transported, assigning a unique LAN (Laboratory Accessioning Number), and completing any additional data input not provided by an electronic chain of custody system.

Screening involves a preliminary swift assessment for drugs of abuse. Although Screening is an economical way to rule out drug usage for most samples, a positive screen needs to be substantiated for court validity. Samples identified as presumptively positive during Screening need a subsequent confirmation.

If a sample is tentatively positive at the Screening phase, additional hair is extracted from the initial sample and prepared for Extraction. Here, drugs are retrieved from hair at significantly lower concentrations than in other methodologies (e.g., urine or oral fluid), making hair drug screening one of the most complex methodologies to execute.

Confirmation of any positive screening result is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a washing process before confirmation if necessary. The complete laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
