A sample measuring 1.5 inches, comprising roughly 200 hair strands (around the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, perfect for screening and confirmation purposes. For EtG, additional tests, and panels exceeding 10, it's advisable to gather 150mg of the sample. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair isn't accessible, an equivalent quantity of body hair can be used. Referring to head hair indicates scalp hair alone. Body hair includes all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial handling of a sample within a lab's database. This process verifies proper sealing and shipping of the sample, assigns a unique LAN (Laboratory Accessioning Number), and completes any necessary data entry absent from an electronic chain of custody system.

Screening provides a preliminary rapid assessment for drugs of abuse. While this cost-efficient method can effectively exclude drug use in most samples, any positive screen must undergo confirmation to be valid in court. Samples that are presumptive positives in the Screening stage require an additional confirmation.

When a sample results in a presumptive positive during Screening, additional hair is extracted from the original sample for further processing in the Extraction stage. Here, drugs are retrieved from the hair at lower concentrations compared to other techniques (i.e., urine or oral fluid), distinguishing hair analysis as the most complex technique to execute.

Confirmation of a positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptive positive undergo washing before confirmation if necessary. The entire lab procedure from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair designation and accredited to ISO / IEC 17025 guidelines.
