Hair testing is increasingly regarded as an effective method for identifying drug and alcohol consumption. Hair captures a long-term record of substance use by embedding biomarkers in the hair strand's fibers as they grow. When strands are collected close to the scalp, they can reveal drug and alcohol use for up to around 3 months. Hair collection is easy, hard to tamper with, and convenient to transport.
A sample segment of 1.5 inches, approximately 200 hair strands in diameter (similar to a #2 pencil), nearest the scalp yields 100mg, which is ideal for both screening and confirmation. For EtG, additional tests, or tests exceeding 10 panels, a 150mg sample is suggested. Using a jeweler’s scale to weigh the specimen is advisable. If scalp hair is unavailable, a similar amount of body hair can be used. When mentioning head hair, it specifically refers to scalp hair. Body hair refers to all other types, including facial and axillary hair.
Process Overview
The process of handling a drug test result in a lab includes four critical steps: Accessioning, Screening, Extraction, and Confirmation.
Accessioning entails integrating a sample into a lab's system. This process checks that the sample was sealed and dispatched properly, allocates a unique LAN (Laboratory Accessioning Number), and completes any additional required data entry that an electronic chain of custody system does not provide.
Screening performs an initial cursory examination for drugs of abuse. Although Screening efficiently eliminates drug use in most samples, any positive findings require verification to be considered valid in legal contexts. Any samples flagged as presumptive positives during Screening need an additional confirmation.
If a sample marks positive in Screening, more hair from the original sample is prepared for Extraction. During this phase, drug residues are extracted at much lower concentrations than in other methods (e.g., urine or oral fluid), rendering hair drug screening the most intricate testing procedure.
Confirmation of positive Screening outcomes is done through GC/MS, GC/MS/MS or LC/MS/MS. Necessary presumptive positive samples are cleaned before confirmation. The entire process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the accreditation for ISO / IEC 17025 compliance.
