What it is, How it works

Hair captures a prolonged history of alcohol and other substances by embedding biomarkers in the fibers of the hair as it grows. When gathered near the scalp, hair can offer a detection span of up to about 3 months for alcohol and drugs alike. Hair collection is straightforward, relatively challenging to alter, and convenient to transport.

A 1.5-inch sample comprising roughly 200 hair strands (about the thickness of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal sample for analysis and confirmation. For EtG, additional testing, and/or evaluations over 10 panels, 150mg of the specimen is advisable. We advocate weighing the specimen with a jeweler’s scale. If scalp hair is not available, a comparative quantity of body hair can be collected. When mentioning head hair, we are specifically referring to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Overview of the Process

The four primary steps in the laboratory procedure for processing a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a sample within a laboratory’s system. This step includes confirming that the sample was securely sealed and shipped, attributing a random LAN (Laboratory Accessioning Number), and completing any other data input not offered by an electronic chain of custody system.

Screening comprises an initial rapid assessment for drugs of misuse. Although Screening is a cost-efficient approach to rule out drug consumption in most samples, a positive screen requires confirmation for legal admissibility. Any samples that are preliminarily positive in Screening necessitate a secondary confirmation.

If a sample is preliminarily positive during the Screening phase, additional hair is extracted from the original sample and prepped for Extraction. During this stage, drugs are extracted from hair at a significantly lower concentration compared to other methodologies (e.g., urine or oral fluid), which is what makes hair drug screening the most intricate methodology to execute.

Confirmation of any positive screening outcome is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All preliminarily positive samples undergo a washing process prior to confirmation as necessary. The comprehensive lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
