For EtG, additional panels, and tests exceeding ten panels, it's advisable to have at least 150mg of the sample. Using a jeweler’s scale to measure the sample is recommended. If scalp hair isn’t accessible, a similar quantity of body hair can be used. "Head hair" refers specifically to scalp hair, while "body hair" encompasses all other types (facial, axillary, etc.).

Process Overview

The process of lab processing for drug test results includes four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a lab's workflow. This step includes ensuring the sample was sealed and shipped right, generating a LAN (Laboratory Accessioning Number) randomly, and completing any necessary data entry absent from an electronic chain of custody system.

Screening serves as a preliminary assessment for drugs of misuse. Screening is cost-efficient for eliminating drug use in most samples, yet legal validation requires any positive result to be confirmed. Samples that are initially positive in Screening need a follow-up confirmation.

For samples with positive results during Screening, additional hair is gathered from the original sample for Extraction. Drugs are extracted from hair in significantly lower concentrations than in other tests (e.g., urine or saliva), making hair drug screening notably laborious.

Confirming a positive screening result involves GC/MS, GC/MS/MS, or LC/MS/MS. All preliminary positive samples undergo cleaning before final confirmation as necessary. The entire lab procedure from Accessioning to Confirmation is evaluated under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
