When collected near the scalp, hair can indicate a detection span of up to roughly 3 months for alcohol and various drugs. Hair is straightforward to gather, somewhat resistant to tampering, and easy to transport.

A 1.5-inch portion of roughly 200 hair strands (comparable to the diameter of a #2 pencil), nearest to the scalp, will provide 100mg of hair, which is the optimal sample size for screening and verification. For EtG, add-ons, and/or tests exceeding 10 panels, a specimen of 150mg is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair is inaccessible, a comparable quantity of body hair can be collected. When discussing head hair, it specifically refers to scalp hair only. Body hair encompasses all other types of hair (facial, underarm, etc.).

Process Overview

The four principal stages in the laboratory processing of a drug test result consist of Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial integration of a sample into a laboratory's system. This step includes confirming the sample was sealed and transported correctly, designating a random LAN (Laboratory Accessioning Number), and completing any extra data input not supplied by an electronic chain of custody system.

Screening entails a preliminary quick assessment for substances of abuse. Though Screening offers a cost-effective method to rule out drug usage among the majority of samples, a positive screen requires confirmation to be legally defensible. Samples that are tentatively positive in Screening necessitate a secondary confirmation.

Should a sample test presumptively positive during the Screening phase, additional hair is extracted from the original specimen and readied for Extraction. At this stage, drugs are eluted from hair at much lower concentrations compared to other methodologies (e.g., urine or oral fluid), making hair drug testing the most complex method to execute.

Any positive screening result undergoes Confirmation via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as necessary. The entire laboratory process, from Accessioning to Confirmation, is conducted under both the CAP (College of American Pathologists) Hair classification and compliance with the ISO / IEC 17025 standards.
