What it is, How it works

Hair testing is recognized as an effective means for identifying drug and alcohol consumption. Hair provides a prolonged record of alcohol and drug use by embedding biomarkers within the fibers of the growing hair strand. When obtained near the scalp, hair can offer an approximately 3-month detection period for alcohol and other substances. Hair collection is straightforward, relatively secure against tampering, and easy to send.

A sample measuring 1.5 inches consisting of roughly 200 strands of hair (around the diameter of a #2 pencil) nearest to the scalp will yield 100mg of hair, which is the optimal amount for assessment and verification. For EtG, additional checks, and/or tests exceeding 10 panels, 150mg of the hair sample is advisable. We suggest measuring the sample using a jeweler’s scale. If scalp hair isn't available, an equivalent quantity of body hair can be gathered. When mentioning head hair, we mean scalp hair exclusively. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

There are four core phases in the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory's workflow. This includes ensuring the sample was sealed and transported correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any additional data input absent from an electronic chain of custody system.

Screening involves an initial swift assessment for drugs of abuse. While Screening offers a cost-efficient approach to rule out drug use for the majority of samples, a positive screen necessitates confirmation to be legally valid. Any samples that return a presumptively positive Screening need further confirmation.

Should a sample return a presumptively positive result during Screening, additional hair is extracted from the initial sample and prepared for Extraction. In this process, drugs are withdrawn from hair at significantly lower concentrations than other methods (such as urine or oral fluid), making hair drug screening one of the most intricate methodologies to implement.

Confirmation of any positive screening findings is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The complete lab process from Accessioning to Confirmation undergoes review under both the CAP (College of American Pathologists) Hair qualification and the ISO/IEC 17025 accreditation standards.
