If scalp hair isn’t accessible, an equivalent quantity of body hair may be obtained. When we mention head hair, we only refer to scalp hair. Body hair denotes all other hair types (e.g., facial, axillary).

Process Overview

Four key stages are involved in the laboratory processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the sample’s preliminary processing into the laboratory’s framework, encompassing verification of proper sealing and shipping, allocation of a random LAN (Laboratory Accessioning Number), and completion of any extra data input absent from an electronic custody chain system.

Screening comprises the initial swift drug of abuse check. Although Screening is a budget-friendly approach to exclude drug usage in most samples, positive screen results require confirmation for court validity. Any samples showing presumptive positivity in Screening necessitate further confirmation.

Should a sample show presumptive positivity in the Screening phase, additional hair is taken from the original specimen for Extraction. In this phase, drugs are removed from hair at much lower concentrations than in alternatives (e.g., urine or oral fluid), making hair drug screening particularly challenging.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing as needed before confirmation. The complete laboratory procedure, from Accessioning to Confirmation, is scrutinized under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
