For optimal screening and confirmation, a 1.5-inch hair segment consisting of around 200 strands (akin to the size of a #2 pencil) from the scalp yields 100mg of hair, the preferred amount for testing. For EtG, extended panels, or tests exceeding 10 panels, a 150mg sample is advisable. We suggest using a jeweler's scale to weigh the sample. If scalp hair cannot be used, a comparable amount of body hair is an alternative. When mentioning head hair, we specifically mean scalp hair. Body hair encompasses other types, such as facial or axillary hair.

Process Overview

There are four principal stages in laboratory drug testing: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the preliminary phase where a sample is integrated into a lab's system. This process includes ensuring the sample's seal and proper shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any remaining data recording that an electronic chain of custody system may not cover.

Screening serves as an initial rapid assessment for drugs of abuse. Though Screening is a cost-effective strategy to exclude drug use in most samples, a positive result necessitates confirmation to be deemed valid in legal proceedings. Samples that return a presumptive positive result in Screening do require further verification.

Should a sample indicate a presumptive positive during Screening, additional hair is removed from the original amount for Extraction. During this phase, the drugs are isolated from the hair at a much lower concentration compared to other methods (such as urine or oral fluids), highlighting the complexity of hair drug screening.

Confirmation of any presumptive positive results occurs through GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive outcomes are washed before confirmation if required. The complete laboratory sequence from Accessioning to Confirmation is scrutinized based on CAP (College of American Pathologists) Hair standards and ISO/IEC 17025 accreditation.
