A sample measuring 1.5 inches consisting of about 200 hair strands (approximately the girth of a #2 pencil) nearest the scalp yields 100mg of hair, the perfect specimen for screening and confirmation processes. For EtG, additional tests, and/or panels exceeding 10, 150mg of the hair specimen is advised. We suggest weighing the hair using a jeweler’s scale. Should scalp hair be inaccessible, an equivalent amount of body hair can be collected. When addressing head hair, it specifically pertains to scalp hair. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result entails four pivotal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary steps of processing a sample in a laboratory's system. It involves ensuring the sample was sealed and transported correctly, allocating a random LAN (Laboratory Accessioning Number), and completing any further data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for drugs of abuse. Though Screening is cost-efficient to eliminate drug use for most samples, any positive screenings require verification to stand in court. Samples with presumptive positives in Screening necessitate a follow-up confirmation.

For any sample that shows a presumptive positive at the Screening stage, more hair is extracted from the initial specimen and prepared for Extraction. In this phase, drugs are extracted from hair at a significantly lower concentration than in methodologies such as urine or oral fluid; thus, making hair drug screening the most challenging method to conduct.

GC/MS, GC/MS/MS, or LC/MS/MS confirms any positive screening result. All presumptive positive samples undergo a wash before confirmation if necessary. The entire lab process, from Accessioning to Confirmation, is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
