What it is, How it works

Hair analysis has become recognized as a robust method for identifying substance and alcohol consumption. Hair captures a continuous record of alcohol and narcotics by embedding biomarkers within the structure of the hair as it grows. When harvested from near the scalp, hair can offer an approximate 3-month window to detect alcohol and drugs. It is straightforward to gather, relatively resistant to tampering, and convenient for mailing.

A specimen measuring 1.5 inches with roughly 200 hair strands (roughly the thickness of a #2 pencil) close to the scalp provides 100mg, which is ideal for testing and verification. For EtG, additional tests, and/or testing beyond 10 panels, 150mg is advised. It's suggested to measure the specimen using a jeweler’s scale. In the absence of scalp hair, a similar amount of body hair may be taken. When we mention head hair, we mean exclusively scalp hair. Body hair encompasses all other forms of hair (facial, axillary, etc.).

Process Overview

The key steps in laboratory processing of drug test results include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the sample's initial incorporation into a lab’s system. This comprises checking that the sample was sealed and dispatched correctly, assigning an arbitrary LAN (Laboratory Accessioning Number), and fulfilling any extra data input absent in an electronic chain of custody system.

Screening performs a preliminary rapid test for illicit substances. Although Screening is a budget-friendly method to negate drug use in most samples, a positive outcome must be substantiated for court acceptability. Any samples deemed presumptively positive during Screening necessitate a follow-up confirmation.

If a sample indicates presumptive positivity in Screening, additional hair is extracted from the original sample for Extraction preparation. Here, substances are separated from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), making hair drug testing the most complex method to administer.

Verification of any positive screening is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive specimens undergo washing before confirmation as required. The entire laboratory procedure, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair classification and compliance with ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
