A hair sample measuring 1.5 inches with around 200 hair strands (roughly equivalent to a #2 pencil) near the scalp yields 100mg, considered the optimal sample size for both screening and confirmation. For EtG, additional tests, or those beyond 10 panels, 150mg is advised. Using a jeweler's scale for precise measurement of the specimen is recommended. If scalp hair is not available, a similar quantity of body hair is acceptable. When we mention head hair, we mean scalp hair exclusively. Body hair pertains to all other hair types (facial, axillary, etc.).

Process Overview

The drug test processing in a laboratory involves four major steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of the sample within a lab’s workflow. This step involves ensuring the sample was properly sealed and delivered, assigning it a unique LAN (Laboratory Accessioning Number), and inputting any additional details not included in an electronic custody system.

Screening constitutes an initial rapid assessment for drug presence. Though Screening offers a cost-efficient means to exclude drug presence in most cases, any positive result necessitates further confirmation for legal acceptability. Samples with indicative positive results at the Screening stage require subsequent confirmation.

Should Screening indicate presumptive positives, additional hair is extracted from the initial collection for the Extraction phase. Here, drugs are retrieved from hair at significantly lower concentrations compared to methods like urine or oral fluids, marking hair drug screening as the most intricate methodology.

Confirmation for any positive results involves GC/MS, GC/MS/MS, or LC/MS/MS. All samples that test presumptive positive undergo a washing process before confirmation is performed as needed. The entirety of lab procedures from Accessioning to Confirmation is subject to review in compliance with the CAP (College of American Pathologists) Hair specification and accreditation under ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
