What it is, How it works

Hair offers a comprehensive timeline of alcohol and substances by embedding biomarkers within its fibers as it grows. When taken near the scalp, hair can provide an approximate 3-month overview for detecting alcohol and substances. Hair is easy to gather, relatively hard to tamper with, and straightforward to transport.

A sample of 1.5 inches comprising around 200 hair strands (roughly the thickness of a #2 pencil) closest to the scalp yields 100mg of hair, the preferred sample for preliminary and confirmatory testing. For EtG, additional testing, and/or analyses exceeding 10 panels, 150mg of the sample is suggested. We advocate measuring the sample with a jeweler's scale. If scalp hair isn't available, an equivalent quantity of body hair can be used. By head hair, we mean only scalp hair. Body hair comprises all other varieties (facial, axillary, etc.).

Process Overview

The main stages in laboratory processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial handling of a sample into a lab’s system. This encompasses confirming that the sample was sealed and shipped appropriately, allotting a random LAN (Laboratory Accessioning Number), and fulfilling any extra data entry not covered by an electronic chain of custody system.

Screening entails a preliminary rapid evaluation for drug misuse. Screening is a cost-efficient method to dismiss drug usage in most samples; however, a positive screen requires confirmation to be valid in legal contexts. Samples identified as presumptive positives in Screening necessitate subsequent confirmation.

Should a sample test as presumptively positive during Screening, more hair is retrieved from the original sample and prepared for Extraction. This stage involves drug extraction from hair at concentrations much lower than those in other methods (e.g., urine or oral fluid), which is why hair drug screening is considered the most complex method to perform.

Confirmation for any positive Screening result uses GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleaned prior to confirmation as necessary. From Accessioning through Confirmation, the entire lab process is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
